Fort HealthCare has announced the members of its 2025 Patient & Family Advisory Council (PFAC). This group aims to ensure that patients, families, and community members have a voice in shaping healthcare services and improving patient experiences.

The PFAC for 2025 includes Andy Yocom and Delores Burington from Fort Atkinson, Nancy Baines who volunteers at Fort HealthCare, Bryan Carrera-Ramos who is an employee at Fort HealthCare, Britanie Peaslee from Lake Mills, Monica Los from Whitewater, and Lesli Rumpf from Cambridge. The chair of the PFAC is Molly Prater.

The council acts as a link between the community and Fort HealthCare. It promotes collaboration to improve care delivery, patient satisfaction, and community outreach. By working with council members, Fort HealthCare seeks to enhance the healthcare experience by integrating feedback directly from those who use its services.

“The Patient & Family Advisory Council embodies Fort HealthCare’s commitment to patient-centered care and meaningful community engagement,” said Molly Prater. “We are grateful to have such a diverse group of individuals who are passionate about making a positive impact on healthcare for everyone in our region.”

The PFAC meets regularly throughout the year to address areas such as care improvement, patient safety, communication, and overall patient and family satisfaction.
