Testing Instructions
Pre-Registration is Required
General Hours
Time:
Monday-Friday
9:00 A.M - 1:00 P.M
1:30 P.M - 3:00 P.M
1. Pre-register online for the COVID-19 PCR test
2. Establish your patient portal as instructed in this packet or link.
3. Go to the District Office at 111 Dodge Street during the above open testing hours
4. Call (920) 206-3334 and our test collector will come out to your vehicle to perform the test.
5. Check your patient portal for results.
6. When you receive your results, please contact your building health room assistant or WUSD Nurse Lynn Gilbert.
